Mac is a community pharmacist in Chatham, Ontario with over a decade of experience in retail pharmacy practice. He has a passion for improving pharmacy workflows and patient care through technology, and has developed several digital tools to support pharmacists in their clinical workflows.
Why PharmaNotes?
Community pharmacists deliver complex clinical care in a high-interruption environment. PharmaNotes was built to make structured clinical documentation faster, more consistent, and accessible on any device — without the overhead of a full EMR or the privacy risks of cloud-based note tools.
Every module is designed around one principle: help the pharmacist focus on the patient, not the paperwork.
What PharmaNotes Does
PharmaNotes Web is a browser-based clinical documentation platform for Ontario pharmacists. It provides structured modules for common clinical workflows — helping you collect information, organize your thinking, and generate formatted clinical notes ready to copy into your pharmacy management system.
Current modules include
- DAP (Data–Assessment–Plan) encounter notes
- Hospital discharge medication reconciliation
- AEFI (adverse event following immunization) reports
- Smoking cessation counselling documentation
- Medication adherence calculator (PDC scoring)
- Diabetic supplies and therapeutic substitution directives
What it is not
PharmaNotes is a documentation support tool — it does not replace clinical judgment, does not connect to external health systems, and does not store, transmit, or retain any patient information.

Each pharmacist using PharmaNotes Web is a health information custodian under PHIPA and remains responsible for the PHI they handle through the tool. PharmaNotes Web is an operational support tool — it does not assume custodianship of any patient information. Pharmacists at individual sites should ensure their organisation's Notice of Information Practices references use of electronic documentation tools.

Privacy documents
The following documents are available for download. Pharmacists using PharmaNotes Web at their site are encouraged to keep a copy of the Privacy Policy and PIA on file, and to have all staff using the Medication Adherence Calculator sign and retain the SOP attestation form.
